Athletic Physicals

Sports physicals will be accepted by private physicians or school district doctors.  All potential Dunkirk City School District athletes are required to obtain a sports physical every calendar year according to the New York State Education Department Commissioner's Regulations (CR135.4(c)(7)(i).  The school doctor retains final approval on any physicals performed by a students' private physician.  A copy of the physical must be given to the nurse's office (in-person or via fax machine at 716-366-9411).

If needed, free physicals will take place during the school year by the School Board approved physician. 

A Pre-Participation Form (coming soon) must be filled out prior to your scheduled physical. No student will be allowed to try out for a team without a current sports physical on file with the Health Office. 

Physicals are valid for a 12-month period.
All athletes (fall, winter, spring) are encouraged to receive their physical in June when sessions are scheduled. If a student misses a scheduled sports physical, there will be set dates throughout the year made available for those students who missed the June physicals. These physicals must be scheduled with the school nurse. To obtain a physical, a student must sign up with the school nurse before or after school.  If the physical is obtained through the student’s personal physician, please provide a copy to the Health Office.
