If you have questions about registration or if you are ready to schedule an appointment, please call Bilingual Registration Clerk at 716-366-9300 ext 4401   

The Central Registration Office is located in School Four at 752 Central Avenue, Dunkirk, NY 

 

To expedite your registration experience, please print and complete our student registration forms prior to your registration appointment. 

          Pre-K Registration forms
          K-12 Registration forms

You can pick up a registration packet, complete the forms, and make an appointment. 
You will need to complete a registration packet for each child. 

 

Required Documents:

Please bring the completed registration packet along with copies the following documents:   

  • Birth Certificate or baptismal certificate
  • Parent/guardian ID 
  • Health & Medical information 
  • Immunization records
  • Physicals completed within the last year 
  • Immunization Record and physical for each child
  • Custody documentation if applicable

Other helpful information:

  • Name, address, phone number and fax number of previous schools attended

  • Previous report cards, transcripts and progress reports

  • English as a New Language Records

Once enrollment is complete in the system, the Registration Supervisor will determine and assign your child’s school building. You will be notified via phone call or letter.

Incomplete registration packets or missing required documents submitted may delay the process of enrollment.

 

IF YOUR CHILD HAS AN INDIVIDUALIZED EDUCATION PLAN (IEP) OR 504 PLAN                       

After registering your child in the Dunkirk City School District, records will be requested from the previous school.  Once records have been received, the Director of Special Education will determine your child's placement based on review of the records. 

If you have copies of the following documents, please bring them with you to Central Registration to assist with placement:

  • IEP
  • psychologist report
  • all evaluations
  • health plan/medical concerns
