Student Registration
Registration is by Appointment ONLY
The first step of enrollment is to complete the online link for the current school year.
The second step of enrollment is to make an appointment to submit required documents and sign paperwork. To schedule an appointment, or for more information, please contact the Bilingual Registration Officer Vanessa Escobar, at 716-366-9300 ext *2026
Families new to the District should register their child(ren) as soon as possible to ease transitions and allow adequate time for educational services to be arranged (schedule, classroom placement and transportation - if they qualify).
Required Documents:
Required registration documents for new/returning students to the Dunkirk City School District
One (1) Proof of Age:
- Certified birth certificate or record of baptism
- Note: If a certified birth certificate is not obtainable, then the district will consider other documentation (see below), provided that those documents have been in existence for two (2) years or more:
- Official drivers license
- State or other government issued identification
- School photo identification with date of birth
- Consulate identification card
- Hospital or health records
- Military dependent identification card
- Documents from Federal/State/Local agencies (Department of Social Services, Office of Refugee Resettlement, ets.)
- Court orders or other court issued documents
- Native American Tribal Document
- Records from non-profit international aid agencies and voluntary agencies
One (1) Proof of Guardianship (document that links the child to the parent/guardian)
- Certified birth certificate which includes parent(s) name
- Court orders or other court issued documents
- Federal or state income tax form within one year that lists the child being registered as a dependent
- Note: In addition to the above, a person other than a natural parent, but in parental relation, must present one of the following:
- Court issued legal guardianship papers
- Court order granting custody
- Court appointment as foster parent
- Parental Affidavit provided by the person in parental relationship assuming legal responsibility for the student

Parent/Legal Guardian Photo Identification:
- Valid driver license, learner permit, or non-driver identification card
- Erie County Department of Social Services Benefit Card
- Employee identification card
- Membership card that has not expired
- Birth Certificate Card
Two (2) Proofs of Address (proof of address must include the name of a parent or guardian):
- Valid New York State driver license, New York State learner permit, or non-driver identification card with current address
- Residential Utility Bill (gas, electric, or cable bill) within the last 30 days
- Pay stub within the last 30 days
- Notice of Decision Statement from the Chautauqua County Department of Social Services within the last year
- Documents from Federal/State/Local agencies (Department of Social Services, Office of Refugee Resettlement, IRS, City Housing Authority, etc) within the last year
- Signed residential lease for current year
- Bank Statement within the last 30 days
- Valid voter registration card with current address
- Valid vehicle registration document
- Custody or guardianship papers with current address
Academic Records
- Latest report card for students entering grades 1-8
- Official transcripts for students entering grades 9-12
Up-to-Date Immunization Record
Students are allowed 14 days from the date they start school to submit immunizations before being excluded from school. Refugee students and students from out-of-state are allowed 30 days.)
Proof of Physical Examination
Within the last year (must be submitted to the school within 30 days of the start of school.)
